I would take it more seriously. Over many years of listening to clients (manicurist) talk of relationships, manners are an important factor in the relationship. The more intimate the relationship becomes or more time spent together even if the relationship remains casual, the more important they are, if you are a person who values them (I wish everyone did).
He should have introduced you, it would have taken seconds.

Yes, he could have gone, before ending his conversation, "Oh by the way, this is Kate".

If it happens once or twice, let it go and pretend not to notice.

If it happens a third time, just join in the conversation, "I know, yes he has been SOOO busy. By the way, hi, I'm kate!"

He should get the hint after.
